I AGREE to provide this dog with a loving environment where the dog will be treated as part of the family and be protected from any long term exposure to inclement or adverse weather conditions. I agree to take care of the dog with proper food, shelter, clean water, proper medical care whenever necessary, and love and exercise.

I UNDERSTAND that I am responsible for all medical treatments and vaccinations after adoption.

I AGREE to COMPLY with ALL applicable LOCAL, COUNTY, PROVINCE and FEDERAL LAWS governing ownership, control and custody of animals.

I AGREE to ensure that this dog will NEVER be subject to CRUEL or INHUMANE treatment by anyone that he/she interacts with including tail docking, ear cropping or any type of alterations that are not necessary and are done for cosmetic reasons.

I AGREE to NEVER use the Dog as an AGGRESSIVE GUARD DOG or for DOG FIGHTING or any other activity in which one animal is pitted against another.

I AGREE to allow Tsintah Siberian Husky Rescue to do a home check six months after this adoption is finalized in order to check on the well being of the dog to make sure it is happy and healthy.

I AGREE to notify Tsintah Siberian Husky Rescue if I am UNABLE to keep this dog due to ANY circumstances. I AGREE not to transfer ownership to someone else unless prior consent is given by Tsintah Siberian Husky Rescue. I ALSO AGREE not to surrender this dog to any animal shelter/rescue or give away the animal for free without prior approval from Tsintah Rescue.

Tsintah Siberian Husky Rescue will do their best to take the dog back at ANY TIME should I be UNABLE to provide a home for the dog; HOWEVER, I UNDERSTAND that my donation is NONREFUNDABLE and there will be NO REIMBURSEMENT of the costs if I return a dog to their rescue AFTER the adoption process had been completed.

In the event that I DO NOT comply with the terms of this agreement after adoption has been completed, I AGREE that Tsintah Siberian Husky Rescue may REVOKE this adoption and RECOVER the dog at ANY TIME and at MY EXPENSE and as such I WILL NOT be reimbursed the donation for the adoption. I AGREE to notify Tsintah Siberian Husky Rescue of any address or phone changes.

I UNDERSTAND that although every effort is made to ensure the health of this pet while the pet is with Tsintah Siberian Husky Rescue, we CANNOT GUARANTEE the health of this dog AFTER it has left our rescue. Tsintah Siberian Husky Rescue is NOT RESPONSIBLE for any medical or other expenses incurred by the adopter. Should a health problem or injury arise, I AGREE to seek immediate medical treatment for my adopted dog.

I UNDERSTAND that this dog is NOT to be euthanized (put to sleep) for ANY REASON without the prior WRITTEN CONSENT of a licensed veterinarian or if the dog is suffering/in pain and has no chance to recover.

I UNDERSTAND that

1. Animals are different from human beings in their responses to human actions;

2. That the actions of animals may be unpredictable.

3. That an animal’s behavior may change after it leaves the rescue and accustoms itself to a home or different environment and;

4. The animal may not be housebroken

5. Their may be issues that Tsintah Siberian Husky Rescue was not aware of

I UNDERSTAND that in the event that I were to return the dog to Tsintah Siberian Husky Rescue for such reason as

above, I am expected to furnish proof of my attempt(s) to seek training.

I UNDERSTAND that Tsintah Siberian Husky Dog Rescue takes in strays, owner abandonments and hand-overs, and

pound adoptions. These dogs may have been neglected or abused by previous owners or may have other health or

behavioral concerns about which Tsintah Siberian Husky Rescue is unaware. Also, particulars of age, breeding, prior registration, prior training, etc. may be UNKNOWN and/or UNTRACEABLE.

Tsintah Siberian Husky Rescue makes every effort to understand the personalities of every animal that comes into their care. Tsintah Siberian Husky Rescue does not GUARANTEE or EXPRESS or IMPLY WARRANTIES of ANY KIND regarding this dog. I understand that this dog is taken AS IS.

If I VIOLATE of ALL or PART of this contract this dog will be REQUIRED to be returned to Tsintah Siberian Husky

Rescue.

I HEREBY STATE that I have READ AND UNDERSTAND ALL of the terms contained within this contract and have discussed

its terms and policies with an Tsintah Siberian Husky Rescue representative. By checking this box, I AGREE to be BOUND

by ALL of its terms and policies.

I agree to indemnify, release and hold harmless Tsintah Siberian Husky Rescue, its directors, officers, members, agents and volunteers from any and all losses, fines, claims, damages, expenses and liability, as well as the defense thereof, including attorneys’ fees, known or unknown, arising out of or relating to my adoption, ownership or possession of the Dog, or in enforcement of this contract, not previously enumerated within this contract.

I am fully aware of, understand, and agree to all the terms and conditions of this RELEASE AND INDEMNIFICATION PROVISION, and acknowledge that it is BINDING AND ENFORCEABLE BY LAW.

By checking this box, I agree that I have read and agree to the following terms. If you have any questions or concerns; please discuss them with us.

This Agreement is the ENTIRE agreement between the parties and MAY NOT be CHANGED or MODIFIED except by a writing signed by both parties. In the event that any part of this Agreement is determined to be invalid or unenforceable, such provisions shall not affect the validity or enforceability of any other provision of this Agreement, which shall remain in full force and effect.
